Abstract

The aim of present study was to investigate whether trichlormethiazide had beneficial effects of lowering blood pressure and adverse outcomes in diabetic patients treated with angiotensin receptor blocker (irbesartan). Hypertensive patients with type 2 diabetes mellitus under treatment with a standard dose of irbesartan(100 mg/day)were additionally given trichlormethiazide(1 mg/day). Clinical characteristics including blood pressure and blood examinations of metabolic diseases were determined before and 12 weeks after treatment. Twenty-seven patients(17 women and 10 men, mean age 69.0±8.3)were enrolled in this study, and no patients reported obvious adverse effects. Systolic and diastolic blood pressure after 12 weeks were significantly reduced, from 151.6±16.3 mmHg to 140.4±17.6 mmHg (p<0.001) and from 80.0±11.6 mmHg to 72.7±11.9 mmHg(p<0.001), respectively. The electrolyte including sodium and potassium and metabolic parameters such as HbA1c, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, triglyceride, uric acid, and creatinine had no differences between before and after the treatment. In conclusion, the add on therapy with trichlormethiazide(1 mg/day)significantly decreased systolic and diastolic blood pressure, and had no adverse effects of metabolic diseases, renal function, or electrolyte in hypertensive patients with type 2 diabetes mellitus who were treated with irbesartan(100 mg/day). The additional treatment of trichlormethiazide over irbesartan would be available and safe in diabetic patients.
