Abstract
A 49–year–old man with refractory heart failure complained of fatigue and dyspnea on effort. He underwent cardiac resynchronization therapy(CRT) because of left ventricular ejection fraction of 27% with left bundle branch block. After 1 week, optimization of intracardiac conduction inter vals was carried out using echocardiography. The CRT optimization data by echocardiography were paced AV–delay of 140 msec, sensed AV–delay of 120 msec and LV pace only. These data were consistent with that had been derived in SmartDelay. Thereafter his symptom and cardiac function were dramatically improved. We experienced a case of successful treatment for heart failure by optimization of intracardiac conduction intervals by using SmartDelay.
