高齢者において，肺炎は特に死亡率の高い重要な疾患である。肺炎球菌はその中でも最も頻度が高い原因菌であり，診療の現場でも遭遇することが多い。耐性菌の増加が危惧されているが，ペニシリン系抗菌薬に対しては高用量ならば反応することも多く，あくまでも高用量ペニシリン系抗菌薬の使用を第一に考えたい。
