目的:ミチグリニドとボグリボースの併用の有効性を検討するために,自然発症2 型糖尿病ラットのスクロース負荷後の門脈血インスリン値,末梢血血糖値に及ぼすミチグリニド単独,ボグリボース単独,ミチグリニドとボグリボースの併用の効果を検討した。方法:門脈内に採血用カテーテルを挿入した24週齢のOLETFラットを用いた。17時間絶食したラットにミチグリニド0.3 mg/kg単独,ボグリボース0.02 mg/kg単独ならびに両薬剤の経口投与を行い,直後に2.5 g/kgのスクロース経口負荷を行った。結果:OLETFラットの門脈血におけるインスリン反応は,初期インスリン分泌が消失し,遅延過剰型の分泌パターンを示した。ミチグリニドは,投与後15 分を頂値(14.9 ng/mL)とする早期インスリン分泌促進作用を示したが,投与後120 分以降のインスリン値は対照群と同様に推移した。ボグリボース投与後の門脈血インスリン値は明確な頂値を示すことなく,緩やかに上昇した。ミチグリニドとボグリボースの併用投与は,投与後15分を頂値とする早期のインスリン分泌反応を示したが,その頂値(7.1 ng/mL)はミチグリニド単独投与時に比べて著明に低下した。インスリン曲線下面積(insulinAUC0–120min,insulin AUC0–300min)はミチグリニド単独群でいずれも有意に増加した。併用群ではミチグリニド単独群と比較し,いずれも有意に低値であり,対照群と比較しinsulin AUC0–300minが有意に低値であった。ミチグリニドおよびボグリボース単独群,両薬剤の併用群の血糖曲線下面積(glucose AUC0–300min)は対照群と比較して,それぞれ約25%,13%,33%低値を示した。結論:ミチグリニドとボグリボースの併用投与は,ミチグリニド単独投与と比較し本ラットのインスリン分泌をより正常ラットの分泌動態に近づけた。さらに,ミチグリニド単独投与よりも少ない追加インスリン分泌量で,より効果的に負荷後の血糖上昇を抑制させることが示された。

目的:左室拡張期心不全症例に対する生命予後改善を認めた薬剤の報告はない。また,左室拡張期心不全は高齢者に多く認め,老年人口が増加の一途をたどるわが国で大きな社会問題になってきている。今回,高齢者に忍容性が高い貼付型硝酸薬の高齢左室拡張期心不全症例に対する有用性を検討した。方法:対象は 65 歳以上の心不全症状と狭心症状の両方が疑われる症例の中で,心臓超音波検査で左室駆出率が 50%以上,かつ心臓超音波検査と血漿 BNP 値から左室拡張機能障害を有していると判断した 6 例(男 3 例,女 3 例;平均年齢 75.8±4.5 歳)。貼付型硝酸薬(フランドルテープ 40 mg)投与前後(平均 264.2±132.3 日)の血行動態,心臓超音波検査を用いた拡張機能評価指標,血漿 BNP 濃度,クオリティ・オブ・ライフ(QOL)の評価を比較検討した。結果:貼付型硝酸薬投与前に比べ投与後では左房径が減少する傾向があり(p<0.1),身体機能に関与する QOL が有意に改善した(p<0.05)。結論:左室拡張期心不全を有した高齢者に対して貼布型硝酸薬を投与することで,硝酸薬投与による前負荷軽減作用や左室弛緩障害改善作用により,左室拡張機能障害が軽減し,身体的機能に関与する QOL が改善することが示唆された。

In some fields it is very common knowledge that Tetracyclin and Macroride Antibiotics have an anti-inflammatory effect. There are huge actual applications in respiratory disease. However there is no study in bone and joint field but only a minocycline report for rheumatoid arthritis. We attained 23 excellent cases in non-infectious arthritis by minocycline administration. I report and display the progress of these cases and search the literatures and deliberate these cases. This method can be practiced with low risk and low cost. I hope broad supplementary examination will be done in many facilities.

Purpose:Raltegravir is the first agent of the pharmacological class of HIV integrase inhibitor, and recently approved by Japan Ministry of Health, Labour and Welfare.ART using raltegravir and emtricitabine/tenofovir is listed as preferred regimen for treatment- naïve patients in DHHS guideline. Here we investigated the clinical efficiency,adverse effects and consciousness survey on HIV patients who were newly treated with raltegravir in our hospital. Methods:We retrospectively reviewed the medical records of 33 HIV patients who had been treated with raltegravir for at least 24 weeks. The clinical characteristics collected from the medical charts included the patients age, sex, CD4(+)cell counts, HIV-RNA, other laboratory data and adverse effects. Consciousness survey was carried out based on questionnaire method Results:Among 33 patients, HIV-drug resistance had been developed in 5 patients at the initiation of raltegravir. In 13 patients switching therapy using raltegravir was initiated due to serious adverse effects of previous medication. Thirteen patients were treatment naïve. CD4(+)cell counts were less than 40/μL in 26 patients, and less than 400/ μL in 7 patients Although starting the medication didn’t affect the blood levels of cholesterol andtriglyceride in naïve cases, it reduced the levels in some switching cases. At this time, every patients were successfully treated and no virological failure was confirmed.Serious adverse effect had not been detected. Discussion:Raltegravir was generally well tolerated and adverse effects have been mild and transient. Consciousness survey also revealed that the medication achieved better adherence.
