SmartDelay による心内伝導の至適化で心機能改善が認められた心不全症例
38巻1号(2017);View Description Hide DescriptionA 49–year–old man with refractory heart failure complained of fatigue and dyspnea on effort. He underwent cardiac resynchronization therapy(CRT) because of left ventricular ejection fraction of 27% with left bundle branch block. After 1 week, optimization of intracardiac conduction inter vals was carried out using echocardiography. The CRT optimization data by echocardiography were paced AV–delay of 140 msec, sensed AV–delay of 120 msec and LV pace only. These data were consistent with that had been derived in SmartDelay. Thereafter his symptom and cardiac function were dramatically improved. We experienced a case of successful treatment for heart failure by optimization of intracardiac conduction intervals by using SmartDelay. -

CIEDs遠隔モニタリングデータ自動収集システムの試作
38巻1号(2017);View Description Hide DescriptionBackground:Introduction of remote monitoring system(RMS) for cardiovascular implantable electronic devices(CIEDs) can be highly effective. However, the data would be scattered through RMS operation and CIEDs treatment become complicated. Method:We have developed a system for automatically collecting RMS data indicated on HomeMonitoring(Biotronik) to in –house database (Electronic Health Record(EHR)) and examined the system with a focus on 2 follow –up patients at our hospital. In addition, we also made a closer examination by a comparison of data access time between RMS and EHR among 8 clinical engineering technologists. Wilcoxon signed –rank test was used for statistical evaluation with p <0.05 as significant difference. Result:The success rate of RMS data collection was 97%. Since median value for the data access time was 47.5 seconds for RMS and 15 seconds for EHR, EHR indicated a significant shortening of time(p=0.016). Discussion:Data of remote monitoring system could be checked by minimum workload due to the entirely –automated process. Since integrated data management system is useful for patient management, clinical study, and recall correspondence, demand of CIEDs data collection system with standards has been growing for improving a stable system operation and development efficiency. Conclusion:It is considered that approach for data integration could be essential for further development of CIEDs treatment. -

糖尿病性神経障害に伴う疼痛を有する患者に対するDuloxetine 特定使用成績調査―最終解析結果報告―
38巻1号(2017);View Description Hide Description糖尿病性神経障害に伴う疼痛を有する患者に対するduloxetine(本剤)の特定使用成績調査は,日常診療下における安全性および有効性を確認することを目的として実施した。収集した363 例の調査票のうち,安全性解析対象症例が348 例で,副作用発現率は14.08%(49/348 例)であった。主な副作用は悪心4.31%,傾眠2.59%であり,すでに知られている安全性プロファイルと大きく異なる傾向はみられなかった。有効性評価項目である簡易疼痛調査一覧(BPI)‒疼痛重症度(平均の痛み)およびBPI‒機能障害の程度の各スコア,患者による改善に関する包括印象度,感覚のあるしびれ,感覚のないしびれに関しては,経時的な改善傾向を認めた。以上の結果から,本剤は糖尿病性神経障害に伴う疼痛に対して,日常診療下においても有用であると考えられた。

2 型糖尿病患者の生活習慣に関する調査報告―食習慣・運動習慣について―
38巻1号(2017);View Description Hide DescriptionObjective:To obtain an understanding of the lifestyles of current diabetic patients, we investigated the dietary and exercise habits in such patients with type 2 diabetes. Method:We carried out an internet‒based questionnaire investigation in type 2 diabetes patients who were receiving medical therapy. We obtained a total of 1195 responses, which was considered adequate for the effectiveness of the current study. A concise summary of the investigation items is as follows:①patient background, ②dietary habits and behavior, and ③exercise habits. Results:Questionnaire responses indicated that the lifestyle situations of type 2 diabetes patients varied according to background differences such as gender, age, and BMI status. The reported tendency of 50 or less years old patients to eat heavily at dinner suggested the possibility that such patients were not receiving adequate dietary therapy. In contrast, although elderly patients reported light intake at meals as well as proper exercise, we considered that the intake of simple carbohydrates due to snack consumption could also exert an influence on blood sugar level. Conclusions:Because patient lifestyles vary according to background, gaining an understanding of such lifestyles is important for diabetes treatment. In addition, we regard the development of new techniques to provide positive instruction to patients to be necessary.
