【私の手術論】 てんかん学のススメ─No One Alone
29巻8号(2019);View Description Hide Description今秋主催する日本てんかん学会では, テーマを「よりそうてんかん医療」としました. 最近は新規抗てんかん薬がたくさん出てきましたし, 外科領域でもVNSといった緩和治療も普及し, てんかんは総合的に治療できる疾患になってきました. そういった医療によって解決できるのに, 取り残されている方がまだまだおられます. 医療者として,可能な限りの手段を 駆使して患者さんの治療に当たるのが われわれの責務だと考えます. -

【専門医に求められる最新の知識:脳腫瘍】 覚醒下手術による supratotal resectionを目指したlower grade gliomaに対する手術 戦略
29巻8号(2019);View Description Hide DescriptionLower grade gliomaは,言語・運動野および高次脳機能にかかわるeloquent area 近傍に しばしば発生するため,脳機能を温存しながら安全に可及的な腫瘍摘出を行うことが重要である. 特にWHO grade Ⅱ gliomaに対しては,最大限の腫瘍摘出を行うことが,患者の無増悪期間お よび生存期間の延長につながると報告されている.そのために覚醒下脳機能マッピングを用いて, 腫瘍境界より外の正常脳組織の機能(言語機能,運動機能,高次脳機能等)を捉えることにより, その部位をfunctional boundaryとする拡大腫瘍切除(supratotal resection)を目標とすべき である.画像上の腫瘍切除だけを目的とするのではなく,脳機能評価に基づいたsupratotal resectionを目指す手術戦略は,次世代グリオーマ手術として重要な位置づけとなっていくと考える. -

【投稿論文:Original Article】 器質化慢性硬膜下血腫の治療─ 局所麻酔下小開頭術の治療成績─
29巻8号(2019);View Description Hide DescriptionObjective: Organized chronic subdural hematoma (OCSDH) is a subtype of chronic subdural hematoma (CSDH), most parts of which contain non-fluid organized clots with a thick membrane. As burr hole surgery for OCSDH usually achieves unsatisfactory results, craniotomy may be required. The efficacy and safety of mini-craniotomy under local anesthesia for OCSDH were retrospectively evaluated. Material and Methods: Between January 2016 and June 2018, 7 patients (male, n = 5; female, n = 2; mean age, 84 years) underwent mini-craniotomy with the removal of most organized clots and the partial removal of the outer membrane of OCSDH. Six operations were performed under local anesthesia and sedation, while 1 was performed under general anesthesia. Mini-craniotomy followed several unsuccessful burr hole surgeries in 5 patients, while mini-craniotomy was performed as the initial treatment in 2 patients. Results: Six of 7 patients achieved good clinical and radiological improvement, and had no recurrence of OCSDH, while 1 patient required a repeated mini-craniotomy with trans-arterial embolization of the middle meningeal artery. The mean operative time was 87 min (range: 55-140 min). There were no operative complications. Conclusion: Mini-craniotomy under local anesthesia could achieve the removal of most organized clots and the partial removal of the outer membrane, and may be an effective and safe treatment for OCSDH. The complete removal of thick membranes of OCSDH may not always be required. -
【投稿論文:Case Report】 海綿静脈洞部内頚動脈瘤の破裂に 伴う頚動脈海綿静脈洞瘻の1例
29巻8号(2019);View Description Hide DescriptionWe report a case of direct carotid cavernous fistula (CCF), resulting from rupture of a known right transitional cavernous aneurysm. A 87-year-old woman, whose transitional cavernous carotid aneurysm had been managed conservatively for two years, presented with sudden headache and loss of appetite. Her computed tomographic scans revealed acute subdural hematoma along the tentorium cerebelli and the retroclivus, as well as a small amount of subarachnoid hemorrhage in the right cisterna ambiens. Carotid angiograms showed a high-flow right CCF which drained into the bilateral superior petrous sinuses, the bilateral inferior petrous sinuses and the bilateral pterygoid venous plexuses. Retrograde intracranial refluxes to the bilateral sphenoparietal sinuses, the right tentorial sinus and the right basal vein of Rosenthal were also demonstrated. Cone-beam CT confirmed a wide communication between the aneurysm and the cavernous sinus, suggesting a rupture of the transitional cavernous aneurysm. Balloon test occlusion (BTO) was performed, and synchronicity of the bilateral cortical vein opacification was observed. We adopted a strategy of parent artery occlusion, because the fistula was so large that its obliteration might not be achieved by a simple endovascular technique of intra-aneurysmal embolization. We performed trans-arterial embolization, and completely obliterated the CCF and the right carotid artery, using a double catheter technique. When treating the CCFs, high-flow fistulas resulting from rupture of pre-existing aneurysms deserve special attention. Among various treatment strategies, parent artery occlusion may be an useful treatment option for direct CCF in such cases.
