長い室房逆行性伝導により反復性非リエントリー性室房同期（Repetitive Non–Reentrant Ventriculoatrial Synchrony）が回避困難であった1例

Abstract
症例は62歳,女性。現病歴:arrhythmogenic right ventricular cardiomyopathy:ARVC。致死性不整脈予防のため植込み型除細動器(ICD)植込みとなる。ICD植込み後不応期約430ms の室房逆行性伝導(逆行性P)により反復性非リエントリー性室房同期(repetitive non –reentrant ventriculoatrial synchrony:RNRVAS)を認め,正常DDD動作ができずAAIRに設定し終了した。症例のRNRVAS回避方法について5 社5 機種のICD デバイスを対象とし検討した。Medtronic 社製VIP Ⅱシミュレータを使用しRNRVASを再現し回避方法についてシミュレーション実験を行った。
