Abstract
The device monitoring capability of telemonitoring is as accurate as that of face –to –face medical care, and device data is transmitted when alerts (such as device failure including lead and battery, arrhythmia detection, etc.) occur. However, in addition to the transmitted device alerts, it is necessary to make a combined judgment including periodic intracardiac electrocardiogram(IEGM).The purpose of this paper is to report on the usefulness of periodic intracardiac electrogram (IEGM) analysis in telemonitoring. To classify a total of 2576 alerts from May 2020 to May 2021 in 154 patients with CIEDs using BIOTRONIK’s RMS, and analyze the necessity of each alert item. Thirteen(1.0%) events were found to be abnormal in the routine IEGM. The breakdown of events was pacing failure: 7, sensing failure: 4, lead disconnection: 1, and 9 were treated by changing settings. It is difficult to detect pacing failure and sensing failure by detecting device abnormality and arrhythmia. It was possible to detect events such as pacing failure, sensing failure, and lead disconnection by analyzing the periodic IEGM as well.
