Abstract
高齢化や生活習慣病の増加に伴い，心不全の頻度は増加している．また近年は，左室収縮不全のみではなく，収縮能が保たれた心不全も注目されており，心不全の動向も変わりつつある．本稿では，急性呼吸不全を来す病態の１つである急性左心不全について，複雑な病態の把握の手助けとなるクリニカルシナリオやNohria–Stevenson分類について説明し，またガイドラインに基づいた初期治療の概略について述べる．
