アスピリン喘息の主病態はシステイニルロイコトリエン（CysLTs）過剰産生であるが，その機序は完全には解明されていない．アスピリン喘息患者の上下気道に集簇した好酸球やマスト細胞がCysLTs の主要産生細胞とされているが，血小板や好塩基球もCysLTs 過剰産生に関与している可能性がある．
