高齢者に認められる無症候性白質病変は，主に脳小血管病による慢性虚血性変化によって生じる．脳室周囲，深部/皮質下白質，橋に生じ，高度なものは脳卒中のみならず認知症のリスクとなり，特に遂行機能や情報処理速度の低下と密接な関係がある．画像所見上は，ラクナ梗塞や血管周囲腔との鑑別が必要である．
