50巻5号(2022);View Description Hide Description日本では,多くの臨床研究が行われているが,日本の医療機関では短期間に多数の症例を集積する必要があり,また,施設間,地域間のバイアスを失くす観点からも多施設共同での実施が必要である。多施設におけるデータの収集を簡便にするとともに,トレーサビリティを確保するツールとしてElectronicData Capture (EDC)を使用しているのが現状である。現在の具体的なEDC の入力と画像収集の方法の流れは図1 に示すとおりである。EDC を使用する場合,実施計画書にて定義したデータ収集項目を記載したデータシートを作成することが主流である。電子カルテからデータシート,データシートからEDC への転記作業では,これらの作業に多くの時間を要するだけでなく,それぞれの工程に入力漏れや転記ミスのリスクがある(図1上)。また,実際にこの業務を行うCRC や医師の立場では,電子カルテ,データシート,EDC 入力用PCと異なる媒体を行き来して利用する必要があり,こうした煩雑なプロセスにストレスを感じることも少なくない。また画像収集については,データセンターにすべての画像を集積し,画像中央判定を行うような研究モデルの場合,必要な画像を検索し,匿名化を行い,対応テーブルをつくり,記録媒体への書込みを行い,データーセンターへ郵送する,といった一連の作業の必要がある。そのため,EDC 入力と同様に多くの人的,時間的コストがかかるうえ,誤記や誤送といったリスクもある(図1 下)。こうした問題を解決すべく,臨床研究中核病院でもある大阪大学が中心となり構築した大阪臨床研究ネットワーク(OCR-net)で行う臨床研究では電子カルテと連動した臨床データ収集システム(Clinicalstudy Data Collecting System: CDCS)および画像収集システムを活用している。これらのシステムは情報通信技術(ICT)を活用したインフラを利用することで,上述したようなリスクやコストを軽減し,臨床研究の効率化と活性化を図り,安全性と信頼性を確保した臨床研究の実施を推進することを目的とている。本稿では,これらのシステムを用いて構築されたOsaka clinical research network for stroke(OCR-STROKE)研究を題材に,OCR-net の有用性について検証と考察を行った。ような研究モデルの場合,必要な画像を検索し,匿名化を行い,対応テーブルをつくり,記録媒体への書込みを行い,データーセンターへ郵送する,といった一連の作業の必要がある。そのため,EDC 入力と同様に多くの人的,時間的コストがかかるうえ,誤記や誤送といったリスクもある(図1 下)。こうした問題を解決すべく,臨床研究中核病院でもある大阪大学が中心となり構築した大阪臨床研究ネットワーク(OCR-net)で行う臨床研究では電子カルテと連動した臨床データ収集システム(Clinicalstudy Data Collecting System: CDCS)および画像収集システムを活用している。これらのシステムは情報通信技術(ICT)を活用したインフラを利用することで,上述したようなリスクやコストを軽減し,臨床研究の効率化と活性化を図り,安全性と信頼性を確保した臨床研究の実施を推進することを目的とている。本稿では,これらのシステムを用いて構築されたOsaka clinical research network for stroke(OCR-STROKE)研究を題材に,OCR-net の有用性について検証と考察を行った。

Characterization of the relationship between the pharmacokinetics of tolvaptan and the pharmacodynamic effects on urine volume after single and multiple doses of intravenous OPC—61815 and oral tolvaptan―A population pharmacokinetic/pharmacodynamic model―
50巻5号(2022);View Description Hide DescriptionBackground/Objectives OPC-61815, a prodrug of the aquaretic drug tolvaptan, is being developed for administration as an intravenous infusion for patients with congestive heart failure(CHF). We developed a population pharmacokinetic/pharmacodynamic(PK/PD)model to characterize the relationship between the PK of tolvaptan and the PD effect of an increase in urine volume in patients with CHF and fluid retention. Methods Individual post hoc PK parameters were estimated and used to develop the model using a sequential approach. Goodness-of-fit and visual predictive check plots were generated to evaluate the adequacy of the final model. Interval urine volumes with varying OPC-61815 infusion durations were simulated and compared with oral tolvaptan. Results The base PK╱PD model consisted of four compartments: water intake, water in edema, water in plasma, and urine. Relationships between tolvaptan exposure and urine volume on day 1 of administration of OPC-61815 and tolvaptan for all doses were adequately described; thereafter, urine volumes at OPC-61815 ≤16 mg and tolvaptan ≤15 mg doses were adequately predicted, but were under-predicted at higher tolvaptan daily doses. The maximum increase in urine volume occurred faster with an OPC-61815 infusion of ≤1 hour versus ≥6 hours, based on the simulation. Conclusions The effect of tolvaptan on urine volume following OPC-61815 infusion and oral tolvaptan administration at clinical or lower doses was adequately described by a semi-mechanistic PK╱PD model incorporating circadian rhythm under the assumption that tolvaptan inhibits reabsorption of filtrated water. Based on the simulation with different infusion durations, an OPC-61815 1-hour infusion appears optimal. (Jpn Pharmacol Ther 2022;50:765‒79) -
機能性食品ナットウキナーゼ(NK)は新型コロナウイルススパイクタンパクやインフルエンザウイルスヘマグルチニン(HA)タンパクを分解する抗ウイルス作用をもつ
50巻5号(2022);View Description Hide DescriptionRecently, a report was published that extract of the health food natto prevents infection by viruses such as severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and bovine herpes virus 1(BHV-1), and we sought to elucidate the actual antiviral action of this extract. In this study, we assumed that Nattokinase(NK)was the actual factor responsible for the major degradation activity in natto extract. For this purpose, NK contained in natto was first purified from the culture medium of Bacillus subtilis natto. When this purified NK was used to act on recombinant SARS-CoV-2 spike protein in vitro, we proved that the viral protein was hydrolyzed by electrophoresis and Western Blot method using anti-SARS-CoV-2 spike protein antibody. It was also found that recombinant SARS-CoV-2 mutant strain S1 RBD(N501Y)was degraded. On the other hand, NK lost its viral degrading effect after heat treatment(HI-NK)at 100℃ for 10 min. Even in the presence of the serine protease inhibitor AEBSF, its degradation was specifically inhibited. Viral HA, which is involved in influenza virus infection, was also degraded similarly by NK, and its degradation was inhibited by HI-NK treatment. From these findings, it was inferred that NK has antiviral activity to prevent not only novel coronaviruses but also influenza virus infection. (Jpn Pharmacol Ther 2022;50:781‒7) -
米ぬか発酵物含有食品の糖代謝改善作用の検証―ランダム化二重盲検プラセボ対照並行群間比較試験―
50巻5号(2022);View Description Hide DescriptionObjectives The purpose of this study was to investigate the effects of food consisting of rice bran fermented product containing 3-(4-hydroxy-3-methoxyphenyl)propionic acid(HMPA)on postprandial blood glucose control in healthy adults. Methods A randomized, double-blind, placebo-controlled, parallel-group study was conducted in 30 healthy volunteers with elevated postprandial blood glucose. Volunteers were randomly assigned to receive either test food or placebo. Daily intake of the test food for 4 weeks. After 4 weeks, volunteers performed a glucose tolerance test without the test food ingestion and evaluated changes in postprandial blood glucose levels. Results All 30 volunteers completed this study. In the test food group, blood glucose levels 2 hours after the glucose tolerance test were significantly lower than in the placebo food group(P<0.05). Moreover, maximum concentration time(Tmax)of blood glucose in the glucose tolerance test was smaller in the test food group than the placebo group. During the study period, no adverse events attributable to the test food were observed. Conclusion The study revealed that the consumption of the food containing rice bran ferment lowers postprandial elevated blood glucose levels by improving glucose metabolism, rather than by inhibiting glucose absorption in healthy adults.(UMIN-CTR ID: UMIN000044481)(Jpn Pharmacol Ther 2022;50:791‒9) -
紅参熟成エキス含有食品の摂取が健常者の睡眠の質に与える影響―ランダム化プラセボ対照二重盲検並行群間比較試験―
50巻5号(2022);View Description Hide DescriptionObjective The goal of this study was to assess the effects of food containing matured red ginseng extract(MRGE)on sleep quality in healthy Japanese adults with sleep quality problems. Methods Fifty-six participants were randomly assigned to two groups via a computer program in a 1:1 ratio. Furthermore, they were asked to take two tablets of the test food(MRGE-containing food)or the placebo at any time for 12 weeks with water. The assessment items were the OSA Sleep Study MA(OSA-MA), subjective symptoms using the Likert scale, and objective sleep evaluation. Results The efficacy analysis data set was a per protocol set, and 27 subjects were included in the analysis for each group. The primary outcome, which was the sleepiness on rising of OSA-MA, was significantly improved in the MRGE group compared with the placebo group. Further, some of the sub-items of sleepiness on rising(concentration, and feeling of freedom)were improved. In the evaluation of subjective symptoms, it was confirmed that worries at the time of falling asleep were significantly reduced. Moreover, in the evaluation of sleep, there was a significantly deeper sleep in the MRGE group than the placebo group. There were no adverse events or side effects found to be causally related to the test food. Conclusions The 12-week intake of a food containing MRGE was found to improve sleep quality in healthy Japanese adults with sleep quality problems. 肌の乾燥を自覚する健常な成人女性の皮膚粘弾性および角層水分量に対する大豆イソフラボン・ラクトビオン酸含有食品の改善効果―二重盲検プラセボ対照並行群間比較試験―
50巻5号(2022);View Description Hide DescriptionObjective The objective of this research was to investigate the effectiveness of daily ingestion of food containing soy isoflavone and lactobionic acid on the viscoelasticity of skin and the stratum corneum hydration in healthy adult women who were aware of skin dryness. Methods A randomized, double-blind, placebo-controlled, parallel-group study was conducted in 70 participants aged 31 to 65 years. Participants were randomly assigned to receive a test food containing soy isoflavone and lactobionic acid or a placebo once daily for 12 weeks. Hydration of stratum corneum, trans-epidermal water loss, skin viscoelasticity, subjective evaluation of skin conditions and urinary equol and isoflavones levels were evaluated. Results The test product significantly inhibited the decrease in hydration of stratum corneum, the increase in trans-epidermal water loss, and the deterioration of skin viscoelasticity compared to placebo. And, the test product significantly increased urinary equol and isoflavones concentration. In ad-hoc analysis focusing on equol-producing ability, equol-producing ability did not affect the results. Conclusion It was suggested that ingestion of soy isoflavone and lactobionic acid-containing foods for 12 weeks in healthy adult women who are aware of dryness increased skin hydration and maintained skin viscoelasticity due to the promotion of absorption of soy isoflavones. 完熟オクラ種子抽出物の肌弾力に及ぼす影響―ランダム化二重盲検プラセボ対照比較試験―
50巻5号(2022);View Description Hide DescriptionObjective We found that mature okra seeds extract had an inhibitory effect on fibroblast-derived elastase activity. In this study, we examined the effect of mature okra seeds extract on improving skin elasticity in a clinical study. Methods A randomized, placebo controlled, double-blind, paralleled study was conducted for 23 healthy subjects(11 males and 12 females aged from 30 to 65 years). Subjects consumed either placebo or test food(300 mg╱kg of mature okra seeds extract, containing OF4949-Ⅱ)for 12 weeks. The elasticity of the skin was measured with a Cutometer® DUAL MPA, and the parameters of R2, R5, and R7 were evaluated. Results Ingestion of mature okra seeds extract significantly improved R2 and R7 values compared to the placebo group. Conclusion These results suggest that mature okra seeds extract, which inhibits fibroblast-derived elastase activity, improves skin elasticity in healthy subjects. 薄毛に対するLactiplantibacillus plantarum subsp. plantarum N793 株含有食品摂取による有効性および安全性の検討
50巻5号(2022);View Description Hide DescriptionPurpose We conducted an open-label, single-arm study on healthy Japanese subjects(aged 20-59 years)who were aware of thinning hair to investigate the effects of the intake of food containing Lactiplantibacillus plantarum subsp. plantarum N793(N793)on thinning hair. Methods The subjects consumed heat-sterilized N793-containing food for 24 weeks. Efficacy was assessed via hair evaluation using a digital microscope, the number of hair loss, and the Visual Analogue Scale(VAS)questionnaire. Safety was evaluated using blood and urine tests. Values of each item were compared between before intake and 12 or 24 weeks after intake using Dunnettʼs test. Results For hair evaluation using a digital microscope, a significant improvement was observed at 24 weeks after intake compared to before intake for the total number of hairs, mean diameter, number of non-vellus hair, non-vellus hair percentage, number of vellus hair, and vellus hair percentage. For the evaluation of the number of hair loss, although the number of hair loss decreased at 12 and 24 weeks after intake compared to before intake, the difference was not significant. The scores of hair loss in the VAS questionnaire significantly improved at 24 weeks after intake compared to before intake. Similarly, the scores of hair thickness, hair volume, and hairline and part significantly improved at 24 weeks after intake compared to before intake. Regarding safety, although significant differences were observed in several items, they fluctuated within the reference values and were considered to be within the range of physiological fluctuations caused by lifestyle habits. Conclusion These results suggest that the intake of N793 improves thinning hair. Additionally, the intake of food containing N793 was demonstrated to be safe. (Jpn Pharmacol Ther 2022;50:843‒54) -
バナナの長期摂取による日本人肥満者における貧血抑制効果の可能性―ランダム化並行群間比較試験―
50巻5号(2022);View Description Hide DescriptionObjectives The aim of this study was to elucidate the effects of banana intake on obese participants on their blood biochemical markers, body composition and visceral fat volume. Methods A randomized parallel-group comparison study was conducted on 36 obese adults between ages 25 to 44. Participants were randomized into two groups; one group took 120 g of banana every day; another group with no banana nor banana ingredients added to their everyday food, for four weeks. Visceral fat, body composition, serum biomarkers including inflammatory markers were evaluated before and after the intervention term. Results 16 participants were in the banana group, and 15 participants were in the control group. Serum iron showed significant decrease in the control group, ferritin resulted in significant decrease in the control group in intergroup comparison analysis. MCV showed significant decrease in both groups, MCHC resulted in significant increase in both groups. Visceral fat did not show statistical difference in the two groups after intervention. Alkaline phosphatase showed significant decrease in both groups. Conclusions The results suggested that taking banana for a long term may contribute in lowering the risk of iron deficiency anemia in obese people. As obesity is a global pandemic, with recent studies showing anemia risk in that population, and that iron deficiency is the most common nutrient deficiency globally, banana intake may contribute to alleviating iron deficiency in obese population. 卵白ペプチドの過剰摂取における安全性の検討
50巻5号(2022);View Description Hide DescriptionObjective Egg white hydrolysate is a functional food material that relieves fatigue through exercise. We evaluated the safety of overintake of egg white hydrolysate in Japanese individuals. Methods The subjects comprised 22 healthy males and females(males: n=9, females: n=13)who consumed a beverage containing egg white hydrolysate(egg white hydrolysate: 15 g╱day)for 28 days. Hematological tests, blood biochemical tests, and urine tests were conducted before and 14 and 28 days after consumption. Results Consumption of egg white hydrolysate showed neither any abnormal value nor any adverse event. Randomized Controlled Trial of the Effects of Tongkat Ali Intake on Stress Markers and Sleep Quality in Healthy Japanese Adults
50巻5号(2022);View Description Hide DescriptionBackground Tongkat Ali has been used in medical treatments in South-East Asian countries and the health benefits with their products consumption have been well reported, but few studies have reported the effect of Tongkat Ali supplementation on stress markers and sleep quality. Objectives The aim of the present study was to assess the efficacy of the intake of Tongkat Ali supplement, compared with the placebo, over a 4-week period, on stress markers and sleep quality, using the Profile of Mood States(POMS)and Oguri-Shirakawa-Azumi sleep inventory MA version(OSA-MA). Methods We conducted a clinical parallel, double-blind, placebo-controlled trial study to assess the efficacy of the Tongkat Ali supplement on stress markers and sleep quality. 33 healthy adults aged 30 to 60 years were divided by age and body mass index(BMI)at baseline, into the Tongkat Ali group, in which the participants consumed the Tongkat Ali supplements, and the placebo group, in which the participants consumed placebo tablets. Results At week 4, improvements in the score from baseline(week 0)in vigour(VA)and Friendliness of POMS were observed only in the Tongkat Ali group(P<0.01, P=0.03, respectively). A significant improvement in the score between 0 week to 4 weeks in sleepiness on rising using OSA-MA was also observed only in the Tongkat Ali group(P<0.01). Conclusions The study indicated the intake of Tongkat Ali supplements has the potential to improve stress markers and consequently improve the sleep quality in healthy Japanese adults. ギリシャヨーグルトの摂取が体重,体組成,筋力,身体機能,体感に与える影響
50巻5号(2022);View Description Hide DescriptionObjectives We aimed to evaluate the effect of Greek yogurt (partheno®)on body weight, body composition, muscle strength, physical function, and subjective evaluation parameters in healthy young adult women. Methods This single-arm before-after study included 49 young adult women. All participants received one pack of partheno®(10.2 g protein)per day for 12 weeks. The participants were provided with training tubes and were advised to participate in light exercise at their daily living activity level. Body weight, body composition, grip strength, physical function(gait speed and stride length at normal and fastest pace), and subjective parameters were evaluated at 0, 4, 8, and 12 weeks. Questionnaires were administered for subjective evaluation of parameters related to defecation, fatigue, sleep, skin, and muscle fatigue. Results The intake rate of partheno® was 96.0±4.8%. Exercise implementation rate using training tubes was 46.5±28.1%. Body weight and body mass index significantly decreased at 8 and 12 weeks. Body fat mass and body fat percentage significantly decreased at 4, 8, and 12 weeks. Grip strength and gait speed at normal pace significantly increased at 4, 8, and 12 weeks. Stride length at normal and fastest pace significantly increased at 8 and 12 weeks. The subjective evaluation parameters, i.e., defecation, fatigue, sleep, skin, and muscle fatigue, improved significantly. Conclusions The daily intake of partheno® might be effective in decreasing body fat mass and improving muscle strength, physical function, and subjective evaluation parameters(i.e.,defecation, fatigue, sleep, skin, and muscle fatigue). タマリンド・ウコン混合物摂取が日本人成人男女の膝および/または腰の違和感に及ぼす影響
50巻5号(2022);View Description Hide DescriptionObjective To examine the effects of a tamarind and turmeric mixture(TTM)on the knee and╱or back discomfort in healthy Japanese adults. Methods This was a randomized, double-blind, placebo-controlled, parallel-comparison study involving 99 healthy Japanese adults. Subjects were assigned to either low-TTM(150 mg╱day), regular-TTM(250 mg╱day), or placebo group(n=33 each)using a random number generator. For eight weeks, subjects consumed food with either low-TTM, regular-TTM, or placebo. The knee and╱or back discomfort were evaluated using the Japanese knee osteoarthritis measure(JKOM), the Japanese low back pain evaluation questionnaire, the Japanese orthopedic association(JOA)score for osteoarthritic knees and back, and an original questionnaire. Knee and back functions were evaluated using a 30-s chair-stand test and a 10-m walk test. The rate of adverse events and blood tests were used to assess safety. Results The comparison of results between the low-TTM group and the placebo group revealed highly novel findings. Thirty-one subjects were analyzed based on the protocol set in the low-TTM and placebo groups. JKOM total scores increased significantly in the low-TTM group than the placebo group, and the JKOM subitem, “knee pain when climbing stairs,” improved in the low-TTM group. The low-TTM group also showed a significant improvement in the JOA score for the lumbar spine. No adverse safety changes were observed. Conclusions The results of this study indicated that TTM consumption(125 mg╱day)for eight weeks alleviated knee and lumbar spine pain in healthy Japanese adults. じゃがいも由来グルコシルセラミドの摂取による皮膚機能改善効果―ランダム化二重盲検プラセボ対照並行群間比較試験―
50巻5号(2022);View Description Hide DescriptionObjectives To investigate the effects on skin function of glucosylceramide extracted from potato, a randomized, double-blind, placebo-controlled, parallel-group trial was conducted on healthy volunteers who were aware of dryness of the skin. Methods Eighty healthy volunteers(Japanese men and women aged 30-59 years)were randomly assigned to receive either a capsule containing 1.8 mg of glucosylceramide extracted from potato or a placebo for 12 weeks. We conducted various skin evaluations: Transepidermal water loss(TEWL), Skin moisture, Skin elasticity, Visual evaluation by a dermatologist(skin texture, skin symptoms), and Visual Analogue Scale(VAS)questionnaire on the skin conditions. Results TEWL values on the cheek in potato-derived glucosylceramides intake group were significantly decreased at 12 weeks in comparison to the control group. In stratified analysis of the group with TEWL <25 g╱hm2, the effects were confirmed more remarkably. TEWL was significantly improved, and the skin elasticity index R2 was also significantly increased. Furthermore, it was clarified that the subjective symptoms of the“overall skin condition”were significantly improved in VAS questionnaire. In addition, any clinical adverse events were not found throughout the study period. Conclusions The present study suggests that oral intake of potato-derived glucosylceramides improved the skin barrier function through improving TEWL and maintained the elasticity of the skin, thereby subjects felt the improvement of their skin conditions. Bifidobacterium breve N106 含有食品摂取が筋肉に与える影響―非盲検単群試験―
50巻5号(2022);View Description Hide DescriptionObjectives This study evaluated the effects of Bifidobacterium breve strain N106 intake on the muscles of healthy adults. Methods An open-label, single-arm study was conducted on healthy Japanese subjects(aged 45-65 years)who were concerned about muscle wasting. Ten subjects consumed heat-sterilized N106-containing foods for 12 weeks. The efficacy of N106 was assessed by measuring body composition(skeletal muscle mass, basal metabolic rate, protein, body fat mass, body weight, body mass index, muscle mass, body fat percentage, upper extremity muscle mass, trunk muscle mass, lower extremity muscle mass, and body water content); administering Chalderʼs fatigue questionnaire, visual analog scale questionnaire, and subjective symptoms questionnaire; and performing motor performance test(lower extremity muscle strength, grip strength, open-eyed one-leg standing test, 6-minute walk test, 6-meter walk test, 2-step test, and 10 times sit-to-stand test)and hematologic specialty tests(tumor necrosis factor-α, interleukin -6). Results There was a significant improvement in grip strength and 6-minute walk test at 12 weeks after ingestion of N106-containing foods as compared with before intake(P<0.05). No adverse events were attributable to the test foods. Conclusions These results suggest that the intake of N106 improves grip strength and walking ability of healthy adults. 50巻5号(2022);View Description Hide DescriptionBackground It has been well established that water-soluble dietary fibers provide nutrition for microbiota in the large intestine. There are many plants and vegetables that contain a high concentration of inulin, such as burdock and onion, and Japanese people commonly have consumed them for a long time. The effect of inulin supplementation on gut have been well known, but few studies have reported the effect on the scientific basis for the interaction between the intestinal environment and skin physiology. Objectives To investigate changes in the intestinal environment, defecation symptoms and skin quality of healthy adults women experiencing constipation after taking inulin supplements for 4 weeks. Methods We conducted a clinical parallel, double-blind, placebo-controlled trial study to assess the efficacy of the inulin supplementation on intestinal environment, defecation symptoms and skin quality. The 30 healthy adult women were randomly divided into two groups and were given inulin supplements or a placebo for four weeks. Results After taking the inulin supplements for 4 weeks, improvement was observed in the items of “feeling of residual stool”and“smell of stool”in the VAS evaluation of stool properties. In the subjective evaluation of the skin condition, the subjects experienced a statistically significant skin improvement effect in the items of“transparency of skin”, and“makeup adhesion”. Evaluation by a skin quality measuring instrument showed that“transepidermal water loss”, and“elasticity R2”item on the left cheek was significantly improved. The Dermatology life quality index(DLQI)showed that“housework obstacles”was significantly decreased in the test product group. Conclusions The study found that inulin supplement intake provided healthy women with changes in the intestinal environment and improved skin quality, and contributed to human health(. Effects of Ursolic Acid in Seaberry Extract on the Frequency of Daily Urination in Healthy Japanese Subjects
50巻5号(2022);View Description Hide DescriptionBackground We previously demonstrated that the eight-week ingestion of ursolic acid(0.36 mg╱day)in seaberry improved several Kingʼs Health Questionnaire(KHQ)scores, including “Does your bladder problem make you depressed?”, in Japanese subjects with mild urinary dysfunction. A significant improvement was also confirmed in the overactive bladder syndrome score(OABSS)for the item“How often do you have a sudden desire to urinate, which is difficult to defer?”. However, we had not analyze daily records on the frequency of urination, which is required by the Food with Function Claims system in Japan. Objective To examine the effects of ursolic acid on the frequency of urination, we analyzed previous clinical trial data. Evaluations of the frequency of urination are mandatory in applications to the Food with Function Claims system according to the requirements of the Japanese government. Methods We examined daily urination records and statistically analyzed data. Results The daily intake of ursolic acid(0.18 mg╱day)for 8 weeks reduced the frequency of urination on days 43, 54, and 56. Conclusions The intake of ursolic acid(0.18 mg╱day for 8 weeks)reduced the frequency of urination. コロナ死者数の分析
50巻5号(2022);View Description Hide Description新型コロナウイルスはCOVID-19で知られるように,2019 年の最後のほうに登場しました。国内では2020 年2 月頃からでした。すでに2 年以上となりますが,いまだ亜種が出続けて収束を見ておりません。国内の感染者数のグラフをみると,これまで6 つの波が観測されています(Box 1)。ちなみに,波で表現するのは日本独特のようです。ウイルスが変異するにつれて,波は大きくなっていく傾向が見て取れます。細かく見ると,第1・2 波は非常に小さく,第3・4・5 波はやや大きく,第6 波は非常に大きな波になっています。感染力は変異するごとに増していますが,重症化は低くなっていることはご承知のとおりです。死者数のグラフを見ると,こちらも明らかに6 つの波が見て取れます(Box 2)。こちらも感染者数推移と同様に,第1・2 波は小さく,第3・4・5 波はやや大きく,第6 波は非常に大きくなっています。波とともに軽症化傾向となっているのに,死者数が増えているのはなぜでしょうか。感染者数(分母)が増えると,致死率は下がっても,分子の死者数は増えます。感染者数が増えると,その中に高齢者や持病のある方も一定数含まれます。そうした人たちの持病が悪化し,死亡したためと思われます。ワクチンが浸透してきているものの,ワクチン未接種の高齢者などが致命的になったようです。コロナウイルス感染症ですが,ワクチンを打つことで軽症化し,ほぼ風邪と同じくらいとも思えます。治療薬も出てきつつあります。そうなると,これはそれほど恐れるものではないという話も出て当然でしょう。今の2 類ではなく,インフルエンザと同じように5 類でよいという人も出てきています。私もそう思います。保健所を通さなくてもよいので,クリニックの先生方も助かるはずです。その一方で,高齢者や持病のある方は感染に伴い重症化し,それが原因で死亡すると心配する声もあります。逆に,それは自身の持病の自然経過と見ることもできます。はたして,コロナウイルス感染が持病の悪化を助長しているのでしょうか。それを確かめるには超過死亡(Excess deaths),つまり死者数が自然増よりも増えたかどうかを見る必要があります。そこで,2020 年と2021 年の超過死亡について分析してみたいと思います。
